For FY11 Community Grants, Grant Reports are due September 9, 2011 and May 15, 2012.
Final Grant Report Form FY11 - May 15, 2012
Grant Change Form must be submitted to request an extension, budget change, personnel change, or other changes to the grant program. You will be notified when the request has been reviewed and approved.
Logos - as a Komen grantee, we encourage you to use our logo when acknowledging funding for your program. Please contact us via email to request the high-res version of the logo. Remember, all materials with our name and/or logo much be approved by the Affiliate before printing.
